I need simple interactive website.

Purpose of website is to schedule clients for IV fluids therapy which is typical 30-45 min session, should be able to select a product (total 6-10), add ons (total 5-8) and pay online or pay in store.
Website eshould be simple, yet powerful, mobile phone friendly.
1. Need to have capability to schedule in which client can schedule appointment we can control time slots availability.
2. Page should show the options to pick, 8-10 total IV fluids (Drips) to pick from
3. Online questionnaire filling with online signing of consent which can be submitted then to a designated email or linked to cloud storage such as one drive.
