What size does this book cover need to be?

It needs to be in "C" format, it measures 153mm W x 234mm H.

Do you require a spine design with it? If so, what is the width of the spine?

Yes I require a spine design. Assume the book will be 150 - 200 pages.

I am open to anything as long as the nominated colours are used.

I was looking to clarify again on the book size. Typically "C Format" (trade paperbacks) are 135mm x 216mm (5.32" x 8.51"). Those standard numbers do not match up with what you have listed.
