Medical Mini one page site with sign up application

I am in need of a one page: phone and web ready marketing site that will help us convert medical professional/doctors to use our service. We help doctors earn revenue through blood work for their non insurance patients while saving the patients money. Our site is bloodpanel.com
Right now we need a dynamic page that is a long form that explains everything and signs them up so they do not have to go to multiple pages. Sign up portion will be at bottom of page.

Much of the marketing will be coming from email and phone apps so the point of what we do needs to come across quick and let the doctors know they can now earn money and help their patients.
The colors would stay the same, the sign up form on the site would be changed up a bit. I would want to ask some questions based on what kind of practice they are and how many people they see a day.
