Patient Recruitment Presentation for a Clinical Trial

We need a powerpoint designed to recruit patients for a clinical trial. We need 4 slides designed: an opening slide, two body slides, and an end slide. The opening slide should include the question, "Do you suffer from Depression?" in the middle of the screen, but other than that just include the design, graphic, or texture that you develop. The 2 body slides should allow for the title of study (please use “Depression Research Study”) to be across the top and contact information (phone number, URL) to be on the bottom of the slide, either centered or left or right justified. Whether this text (study title and contact information) is set off by a banner or shape that stretches across the entire slide or just provides a background to that text is up to you.
