Hi! I am a clinical pharmacist. This is a business that aims to provide medication management help and advice for older adults, their caregivers as well as other adults. I've named this business: Viva Rx Solutions.

I would be meeting with patients either in person or via telehealth and reviewing their medications, ensuring their regimens are appropriate and optimized for their medical conditions. I would be reviewing their medications to identify any drug related problems including drug-drug interactions, inappropriate medications or doses, polypharmacy as well as medications that should be deprescribed or discontinued. I would provide education on how to take and use their medications. After meeting with the patient and reviewing their medications, I would make recommendations and provide them to the patient, caregiver and provider (doctor).
