Brief de Design Graphique

Hello,

Task is pretty simple just time consuming. Basically I run a nutrition business and i want to convert my paper based client intake forms into digital PDF forms that can be filled out on your computer. This way I can email clients the forms and they can fill them out and email them back, saving paper and time.

All the content has been written, i just need YOU the designer to convert it to a USER FRIENDLY PDF form!
