I do require coding for the website and plan to choose the design later today or tomorrow.  

in terms of the online registration form: I would like a box that patients can request an appointment with fields including name, email, mobile phone number and preferred date of appointment.  This is then submitted to the practice email address.  Thanks again for all the designs.
