Brief de Design Enveloppe

We need the following PDF redesigned to be more user friendly.

We are finding that

1. People are not setting up payments
2. People are not returning the paperwork with the bank information properly filled out.
3. The bank details are actually not used, they are only a backup. This needs to be made prominent. many people phone us about this.
4. People find it all too confusing. Perosnally i think its easy but hey.. im not our customer.

Content can be totaly rewritten,

This form gets automatically filled out by our software, printed and then mailed to them. They only need to sign, and fill out the bank details and return it. They also need to call up to setup payments.

The final ezidebit form can be redesigned however, how too much. eg, Locations of content should be fairly much the same.

Current document is attached. its the PDF.

I have also uploaded a logo, and a .doc which is handy to edit. Also have uploaded the ezidebit PDF too.

Our target market is people who dont have very good english skills, cant read well and need something really easy and simple to follow. Something thats to much effort becomes too hard
