Brief de Design de Papeterie

Hi, I am a dentist/oral surgeon recently graduated and starting my own practice. Need help in designing an attractive, warm, appealing and Professional referral form (double sided), and stationary design. These will be used for Thank you notes, Professional letters, Front of a presentation folder, Notepads, envelopes, promotional products, Fax, etc... I do have a sample of a typical referral form and also have a logo as well as business card design that I will provide as a base to use. I will attach the files to this posting. Please, do some investigating on the Oral surgery / dental specialist websites as to some of the examples of the referral forms. Thanks and feel free to respond with any questions. Look forward to the designs - having the business card designed here was a great experience.
