Brief de Design de T-shirt

We are a busy, growing Optometric Practice (eye doctor) in Brush, Colorado. Our t-shirt printer went out of business, we are going with someone new and want a new shirt design to go with it. We are member of Vision Source (a national group for private docs of optometry). The Vision Source colors are black, gray, white and gold however, we are open to other colors. The previous shirt colors are black and blue but again, we are open to changing the shirt colors. The design needs to be eye catching yet simple so people don't have to figure out who we are. The front needs the Vision Source logo on the left (attached), the back should include our phone number and website address.
