Brief de Design de Carte de Visite

I'd like a business card made using the logo attached.

I’d like the back to be white background with a small version of the logo and the words: Gastroenterology, Hepatology, Diagnostic & Therapeutic Endoscopy


I need the two address locations, the website address and the line that all appointments are through the 96351510 number
And then the bottom ½ or slightly less needs a couple of lines for the next appointment.


I’d like all the text on the card to be modifiable by me in case changes to phone numbers etc is needed later.
