Brief de Design de Flyer

I need the Scorecard I've developed for my staff to be produced into a visually engaging piece that will encourage the staff to utilize it on a daily basis. I'm thinking it could be 5x7 in size so that it isn't too cumbersome, but I'm open to different layouts.
I have multiple other departments that will need Scorecards created for them as well, but each one will contain task specific questions for each department.
