I am an internal medicine physician.
Hospitals that employ internal medicine physicians and insurance companies that include such physicians on their panels require these physicians to be "board certified" by the American Board of Internal Medicine (ABIM). ABIM has no real competition. It is a monopoly.
Its executives are themselves internal medicine physicians and many of them are paid in the high six figures. Until recently, board certification consisted of passing a comprehensive exam every 10 years. But due to the need for more revenue to fund ABIM executives' high salaries and lavish lifestyles, they recently required internal medicine to participate in a useless program called "Maintenance of Certification" (MOC) which consist of a series of self learning modules that ABIM claims are important to keep physicians up to date on latest knowledge. Internal medicine physicians must pay several hundred $ per year to participate in MOC. If they do not, they are considered non-board certified.
