Brief de Design de Papeterie

Hello,

I want to work with a designer for my startup Oral Surgery Practice called: San Fernando Valley Oral Surgery & Implant Center

Ideally I would like to choose a good/professional design and work with a designer all the way to fulfill ALL of our stationary needs including referral pads, business cards, letterheads, envelopes, notepads, email signature, etc.

I would like to see designs first and hopefully would like to work with the designer for the lifetime of my practice and wanting to develop a long-term professional relationship.

In terms of the logo I have attached our practice logo files: I want to stay within the 3 colorways of the logo: Navy Blue/Silver grey/White. I would like for you to be creative and incorporate the "V" as best you can (for example greyed out watermark, etc)

I have also attached in my files the full contact info of our office in a word document: which includes address, email address, phone, fax, cell, etc.

I have also attached a few examples of double sided referral pad we can draw inspiration from. In terms of the referral pad I would ideally like to have my logo nicely placed/incorporated to make it look professional, address of the practice and contact info, my name and credentials.

Referral pad must have lines for patient, date of birth, phone number, referring doctor name, odontogram of all adult teeth and pediatric teeth, nice and big - so easy for dentist to refer. Also a jaw drawing of the intraoral cavity. Below that should have comments to surgeon.

In terms of procedures, I would like to have options of IV sedation, extractions, wisdom teeth, bone graft, pathology, tmj, trauma, expose/bond, orthognathic, other (list below)

In the back we can have a greyed out map of my practice address and Instructions for patients should include to bring insurance documentation, medication list, etc.

The final nitty gritty details of the referral pad can be worked on once I am happy with the design.
