Brief de Design En-Tête

Please create a letterhead design that includes my logo, and options of my light blue/black color scheme and black and gold so I can choose between the two. I would like my Contact information on the top or bottom of the letterhead somewhere:

one that is blank, and one that has a signature spot on the bottom so I can sign:

I am going for classy/professional. I want other doctors and or lawyers to look at my letterhead and want to work with me. Again, professional, classy, clean. I do like black and gold but I could see what the blue like my other business card looks like too. Please do what most people do for letterheads--I'm not sure if the signature line should be on there? Or should be blank? I have no clue
