Brief de Web Design

This will be a place for users to quickly generate numerous forms from a categorized list. As the forms are templates with field variables, the site must be able to substitute ‘real data’ for template variable data. This tool will allow for quick custom forms on-demand.

The landing page is attached. We need the User Experience and User Interface solved for us.

Coming to the Forms Landing Page, the user will see the forms available to select. The user will then click to select a form.

At some point, the user must resolve the variables in the form. For instance, there are the following variables in the above example: PatientFirstName, SelectedDate, SelectedProvider, and SelectedAuthor. We must give the user a chance to select the values for these variables. Using a dropdown combobox will probably work for most. Getting a date selector will be required if the variable happens to contain the string 'date'.

Is it best to preselect common variables before opening the form, or is it best to dynamically scan the template, then give the user the option to give values to each variable found?

The design must include a way for users to upload additional forms. Forms will most probably be PDF, .DOC, .TXT files. This allows users to develop simple or fancy forms of their choosing., then simply drop them into the site for future use.

The design must include a way for users to create new forms. The form creation tool must present to the user the list of predefined variables like {{PatientLastName}} for easy insertion into the form writer. Anytime "{{}}" are used, developers will recognize that as a variable that needs to be resolved.

The forms will be separated into categories as follows:

- Fax Cover Sheet
SCHEDULER:
- Appointment Reminder
- Missed Appointment Policy
PATIENT:
- Intake Form
- Patient Discharge
- Return to Work or School
- Excuse Slip
- Consent to Treat a Minor
- Verify Non-Pregnancy
RELEASE:
- Health Information Release Form
- X-Ray Release Form
CERTIFICATES:
- Certificate of Care
- Disability Certificate
INSURANCE:
- Authorization to Bill Insurance
- Assignment Authorization
OTHER:
- Free Massage
- Free Vitamins

The design must include a way to add/customize categories. Categories are for organizing the forms logically.
