Can you get all copy for each page as selectable text.
Can you provide the logo
Can you provide a flow chart of your user journey.

You can use and browse the current app at: teamboosters.nl.

Flow chart is:

home-> user selects tool with button -> user creates a survey by providing a team name and emailaddress -> app provides a custom link to survey -> user click link and is able to fill out the survey and/or view the results
