Brief de Design de Flyer

Design cover for information booklet

I am looking for a design for the front and back cover of an information booklet. Also an inside page basic design.
The booklet will contain 10 double sided pages providing information on weight loss surgery.

See my website www.anthonyclough.com.au for what I do and typical colour themes.

I would also like some theming/illustration which can be utilised on each inside page of the booklet, rather than the pages just being blank with text etc. What would a blank page on this inside of the booklet look like under the design theme that you have created?

My company is Melbourne Centre for Bariatric Surgery. You will find uploaded a Logo for Melbourne Centre for Bariatric Surgery as well as my other logo which is my name “Anthony Clough, etc.”. The Melbourne Centre for Bariatric Surgery logo should be utilised. The other logo (Anthony Clough) is optional.

The front cover should contain the following text as minimum:

Melbourne Centre for Bariatric Surgery
Mr Anthony Clough MBBS FRACS GradCert (Health Stats)

Our tagline is “Experience, Expertise and Empathy”. This should probably be on the front cover also.

The back cover contains (as a minimum) our address, phone number, fax number:
