Brief de Print Design

At Home Sleep Studies main marketing tool is our Prescription that we leave with medical providers. Prescription is printed on 8.5 x 11 sheets of paper to create a prescription pad for our medical providers. Prescription should fit to one page. At Home Sleep Studies works with 250 different medical providers so it is important we are able to update and change the medical provider information at bottom. The medical provider would fill-out top with patient information, check-off center testing and patient co-morbidity and Fax to At Home Sleep Studies. Because it is Faxed to us it is important to have clear print and large font Attached or uploaded file is one example we currently use. Please review all 5 must haves.

• 5) Document Must be created in Microsoft Word 2016. This is the program we use for our prescriptions.
