Brief de Design de Brochure

Please help us design a brochure for our company, Dental Education Partners, LLC to use for sales and prospecting!
It is our first brochure, and as we are a small company, we don't normally use them, but we have an upcoming meeting and need a GREAT looking brochure with a call to action!
In addition to a pdf print ready version of the file, we need to receive a version with editable pdf text fields.
Our original thought is a 1-page, 4-panel fold or Tri-fold brochure, but we are open to recommendations based on the content.

I have uploaded our logo files, some images for consideration, and proposed text content for the brochure. Note - if you have the space to add our AGD PACE provider approval we would like to. If you do use the AGD Pace logo, we need to follow their logo guidelines which are also attached.
