Brief de Web Design

We are looking for two things:

1. On our main website (www.pacificspecialty.com) we would like to add a section for someone to enter their address and email address in return for a "quick quote."
2. Once they hit "enter" we would like a page built out that contains all the information that we would plug in from the backend.

We are an insurance company that focuses on property and powersports. This project in particular would focus on property (homeowner's specifically).
