I want this to have graphics and to collect their full name, email address, country they are from. It must say Sign up today and receive all these items for free, A $129 Value, 1. I want this to look exciting so people actually sign up.
I want to be able to put this on facebook and add to my wordpress website.
I want it to match my current website
I currently use ninja forms on my site

Once designed this also needs to be coded for my site.
