1. Do you want each Front page design (12) different with same Theme?

2. Do you want different Foot Note on each Front design? Can I take it from your website?

3. Do you want Previous and next month Calendars on each Front page design?

4. Do you want different Back page design for each similar to Sample 5 ?

The Front Cover Page Design with different wording as photo in Sample 1. Preferable to include the Theme in the Front Cover Page.

Front Page design different for all 12 but with the theme element of Spine, Health and Chiropractic.

2. Do you want different Foot Note on each Front design? Can I take it from your website?
Footnote will be promoting our website. It can be our home page which is www.chiroacm.org or other links with are in our website. Eg. About Chiropractic ([Non image link removed]and so on.

3. Do you want Previous and next month Calendars on each Front page design?
It will be good to be include the previous and next month calendars which shown in sample 3 and sample 4.

4. Do you want different Back page design for each similar to Sample 5 ?
The back page design for each month be different but with the theme element of Spine, Health and Chiropractic.
