Brief de Design de Pochette de CD

Create a CD jacket (front and back). Front artwork will be used on CD itself.The CD is an EP (6 songs). They are all love songs (Ballads). Violin, cello, piano, percussion, acoustic guitar, that sort of sound. I am looking for a sophisticated look to the CD. The title is Finally Home and the subtitle is The Story of Us. The picture is of me; Bo Short. I want this CD jacket to exude class. I have uploaded an example of what i think i want. Obviously rough on my part.

Besides the attached picture of me, I am looking for: my name, name of Cd (Finally Home) and subtitle (The Story of Us)
