Brief de Web Design

We need a website for a Philippines distribution company of Invisalign. The purpose is to redirect the user to the corporate site if they need more information, and to provide the user a list of available doctors in the Philippines. I've attached a word document with a rough outline of the content. Below is a description of the pages needed.

• Home page: Use Invisalign logo on the top and provide a nicer graphical representation of the text. For example, "Philippines" is plain but I would like to look a little nicer graphically, maybe tying in the Invisalign graphic.
• What is Invisalign? This link should open a new tab, keeping the original home page open. Can you make this text graphically nicer?
• Find an Invisalign Doctor (info on page 2-3 of the attachment): Is there a cool way to show the list of doctors and still keep the user on the home page? There are currently about 15 doctors and the list may grow to about 30-40. We will need the ability to change the doctor list content. Can you also make this text graphically nicer?
* Create a category called "Sign Me Up"? It should then allow open a subwindow within this page to open and have the user fill out a form. The form should have a pull down for Dentist or Patient. That way when we receive it, we know how to handle it.
• Create basic company logo for Luna Distribution, Inc (the distribution company) and put on bottom. Logo needs to be available to work on letterhead and the future website design.

I'm okay if you want to add Flash elements to make it more interesting, but it should be designed as a site for high end consumers.
