Brief de Design de Carte

Hello - We looking for a flyer with a card that can be torn off at the bottom for our referral program. The flyer needs to be simple and can basically be business card on top of another with the following information. Look at the files I made a sketch of what I have in mind. We do need images that will fill in the space

Basically we are giving $20 to a person if they refer someone and then that person will receive $20 towards eyewear or an annual supply of contact lenses
