Brief de Design de Papeterie
Design a medical history form for a dental practice. Content is provided so all that is needed is the formatting to make it look great and easy to read.
"Patient Information" to include all information contained in top portion of the "Patient Medical History Form" docx file.
"Medical History" should follow the formatting and content of the "Exact Medical History" file as this is what the entry fields look like in our practice software and so we would like it to match.
1st page - Patient info, allergies, medications, antibiotics, joint replacement, pregnancy.
2nd page - Do you have, or have you had, any of the following? . . . . . . .
