Brief de Print Design
I need an order form created for my landscaping company. It will be used when we go door to door to make sales. This form will be used as a two part carbon-less copy form, so that when we write on the form at the door of a prospective customer we both can keep a copy of the information. The first file attached is a copy of a competitors form and sort of an example of how I would like ours to look. The second file is the writing that I want to incorporate on mine, since it will be a little different than the competitors services. It is not exactly how I want it to look so feel free to rearrange things to look good. I believe how the first example has it, the areas in which we will write things in will be white, and the surrounding areas will be colored. Green is good for the main color scheme. The third file will be the logo I want attached. I have the logo in various other files if you need.

The additional services should also have a small box next to them so that you can check to receive, notes should also be in a box, remember this is an order form and needs to be functional, look to the example as a guide.
