Brief de Design de Brochure

I don't need a full brochure. I need an insert that my clients can put inside their trifold brochures. (finished size should be 3.5" by 8.5") My clients are Hospice Companies and Funeral Homes. They provide our services free of charge to their clients. Our services include helping each family who experiences a death wrap up all the financial and estate matters of their loved one. We will go through a checklist with each family. We first explain what the next steps are such as how Social Security benefits work and what exactly will now happen to their benefits. We then go ahead and call social security for them on a conference call and change the benefits as needed. We do the same thing for everything else they need to do. The areas we cover are Government (Social Security, VA, Railroad, OPM, etc), Insurance to claim benefits for the deceased/ cancel policies/ change policy and beneficiaries for survivor(Health, Life, Long term care, Car, Home, ETC), Financial Institutions (Pensions, Retirement Accounts, 401K's, IRA's, Annuities, Credit cards, Bank accounts), Estate Issues (Car Title, House title, Wills, Trusts, Funeral planning, Taxes), Convenience issues (Stop junk mail, deceased registry, do not call list, Fraud protection, cancel memberships ie AARP, forward mail, etc.).I must have a place to print my client's logo on it where it says "Service provided free of charge by:"
