Brief de Design de Brochure

I have been working with a local designer to produce a mailer for a dentist. I don't think the current look or info is very good. I need a brochure that will go in the mail to a new resident moving into the service area of the dentist. In other words, an information piece that has a call to action such as an offer for cleaning or a free consultation or 2nd opinion. I will provide the current design which I don't like, along with other samples I have received in the mail myself. I am looking for an 8.5 x 11" which will fold to 8.5 x 5.5 for mailing. Fold needs to be on the bottom when looking at the mailing panel. No bleeds if possible. This mailer is meant to be effective and get action (phone call or web visit), I'm not looking for the best-looking design to win awards. We need the recipient to act with a phone call or visit to the web site. If this project meets with the client approval, there will be 3 more offices we will need to produce for.
