Brief de Design de Flyer

We need a trifold brochure that would go out to physicians. I am marketing to them to get patient referrals. My business name is Back to Balance Physical Therapy and Wellness, PC. We specialize in treating balance and dizziness and other neurological disorders as well as in providing general physical therapy services. We could use a logo of some sort on the brochure. Please go to backtobalancept.com and look at the about page. My business does seminars too, but I want this brochure geared to the physical therapy services we provide. The business card I attached has important information and the phone and fax number. My address is:

I would like information on my business, but please include my name also. I am the sole physical therapist of the practice, but want to give the impression I am not just a small operation. I need the brochure to have Back to Balance Physical Therapy and Wellness as the title, but information on me as the owner. I attached the brochure for some ideas. This is the one for my seminars. I don't want the new one geared toward seminars or this busy. However, the brochure does have some idea, like the graphic of the inner ear and then some information on me.
