Brief de Design Graphique

We are in need of an infographic layout to highlight facts for a medical foundation - the Blue Lips Foundation. We are a foundation dedicated to early detection and screening of pulmonary arterial hypertension, a deadly combination heart and lung disease We need a graphic that incorporates the following facts:

(1) Pulmonary Hypertension Symptoms: •Chest Pain
•Dizziness
•Shortness of Breath
•Fatigue
•Edema or swelling
•Cyanosis or skin discoloration

(2) Typically PH patients are consulted by 3 or more physicians before diagnosis

(3) PH patient’s generally incur 2.8 years between symptom onset and diagnosis via right heart catheterization

(4) Left untreated, the average life expectancy of a PAH patient is approximately 3 years from symptom onset

(5) No improvement has been achieved in early detection rates of PH since 1995

(6) 5 year survival rates are measured at 86% for PH patients diagnosed in early stages. 5 year survival rates for PH patients diagnosed late stages decrease to 50%

We want to incorporate these highlights into a fun and easy to follow infographic that can be transferred onto a large scale display banner.

We want to utilize our logo somewhere on the graphic. The logo file is attached. We also want to incorporate our slogan: Igniting the (ph)ight against pulmonary arterial hypertension through early detection and screening.
