Brief de Design de Brochure

I need a creative Podiatrist Brochure designed for my practice. It Requires my name : "Andrew S. Klayman B.Sc.,D.P.M -Podiatrist" on the front page .
with creative design with picture front page that grabs your attention. The back page needs my two office locations and phone numbers with my website address . Look at my website www.askpodiatrist.ca for ideas and content . I want to focus on orthotics/ sports medicine , ingrown nail correction, laser for fungus and plantar warts. I am using this brochure to create interest for patients and referring Doctors on what we do and the solutions we can offer these patient . Brochure should be professional, warm, create confidence with modern clinic feel to the clinic but creative out of the box design.
