Brief de Design de Modèle Word

We are dooing lot of médical report from word document. We dont print the text on a stationnary. We are producting the report and save it in pdf. So I need à word template. I want something more than just hard the logo. I would like the look of a stationnary in the word template.

I put an exemple of report. But the final don't have to look that way. Propose something more acurate.
After that I will need a business card.
