Is there a title or name for your campaign you want to put on your t-shirt?

That is a good question.
At this stage I am just planning on putting something on the back, at most. As this is a post campaign t shirt, it's no so much for advertising as it is a bonus.

Will the design be screen printed? Dye sub? Need to know how many colors we can use in the design.

The response I got was that they are going to be printed and not sublimated, hope that's what you were wanting to know.
I'd suggest trying to keep it to two colours. I am currently trying to get a more in depth cost break down but that is around my current plans.

I do like it a lot more than the first. I've had some good feedback from people that I've shown it too so that is always good :)
