Gift Card Design - Clean and Minimalist

We are looking for a clean and minimalist gift card design for a chiropractic office. It should be a unique design and not too standard. Ideally integrate the logo file into the design in some way.

The gift card design should have the following:
- A field to enter the name of the recipient
- A field that indicates the gift card number
- A field that is interchangeable for either a ''chiropractic adjustment'' or ''new patient visit''. This could also be a checkbox to pick either option.
- include the QR code (this is where the person can make an appointment through the online booking)

The format should be 21cm wide and 9.8cm height.
