MoveSMART observation card

We need a card, business card sized, designed. This card will be given to employees in our warehouse that we identify using safe lifting techniques from the MoveSMART program. One side will be the cover stating Thank you for: Listening, Moving SMART, Staying Safe, Protecting Yourself and include the MoveSMART logo. The other side, the employee will write their name, the person who observed them, and the date. They will also circle what technique they were observed doing. This needs to be printed on a surface where people can write. We would like to see the yellows, greens, and blues that MoveSMART employs. I have a very, very, rough draft that I will attach but I am open. We have to have the TM on certain items that are included. Voir plus
