I'm looking for a rack card or trifold brochure to advertise elective ultrasounds

I need something that looks professional but eye catching to bring to doctor's office for display. I want it to have my logo, some 3D images and have a cohesive design. I want it to give a summary of what I offer in terms of elective ultrasounds: Peace of mind, early gender, 3D/4D. It should also mention that I can travel to the patient for in-home ultrasounds.
