50 Questions Book Cover Design

We need a book cover design for a 6x9" size book we are laying out with createspace.com. They just aren't nailing the awesome design we are after for the cover. The book is titled 50 Questions (You MUST Ask) Before You Have Plastic Surgery. The book is authored by Brenton B. Koch, MD, FACS. He would like an eye-catching, modern, edgy yet sophisticated, cover that utilizes metallic blue and vibrant coordinating colors. Perhaps a beautiful face with scaffolding or something to suggest a work in progress. Dr. Koch is Des Moines, Iowa's, only double-board exclusively facial plastic surgeon.
